Important Forms
The links below will guide students to important forms that may be required in order for the Office of Financial Aid to complete verification and/or appeal processes and to confirm financial aid eligibility. View your Required Documents in the my.Fairfield student portal. Contact our office if you have questions about how to complete these forms, or with questions on any part of the financial aid process. The deadline to submit any/all financial aid requirements is August 1.
Academic Year 2023-2024
Verification Forms
Please note that the Student Title IV Authorization Form allows the student to give the university permission to receive all financial aid funds (federal, state, institutional, other) on their behalf for educational expenses.
Enrollment Verification Form
Change Loan/Loan Adjustment Requests
Use this form to request a new or additional federal loan (Direct or Graduate PLUS) as well as Private Loan adjustments.
Cost of Attendance Appeal Form
Use this form to request an increase to the cost of attendance of your program in order to borrow additional loans that will assist with college expenses.
