- Follow the instructions on the form.
- If you have questions about the waiver, please contact Haylor, Freyer, & Coon at 833-641-6489 or student@haylor.com.
- To qualify for the waiver, the student's health insurance policy must meet or exceed the coverage provided by the University-sponsored plan. The policy must also be written and filed in the United States.
- The following coverages do not meet the University’s waiver requirements and will not be approved:
- Out of State Medicaid plans except NY, RI, MA, CT, NJ (for CT campuses) and TX Medicaid (for Austin campus).

All full-time undergraduate, graduate, and second-degree nursing students are required to maintain health insurance. If the student already has insurance and does not need the University-sponsored health insurance policy, a waiver must be completed.
Please contact student@haylor.com to enroll in the student health insurance plan outside of the open period or with any questions.
The deadline to waive for the 2026-27 academic year is Friday, September 18, 2026.
