Parents/Guardians Checklist
Parents/Guardians, your student has several action items to accomplish within their own online checklist system. In addition, all their action items are listed in the action item section of the First-Year Student Guide, which they will receive early in May.
However, there are specific action items that they may need your help with and/or for you to complete on your own. Please note all of the following action items are part of their online First-Year Student Checklist. If you decide to complete an item below, please ensure that your student has not already done it.
Parent/Guardian Orientation Registration (Due May 31, 2013 - will be available by May 1)
Parents/Guardians are to register for the Parent/Guardian June Orientation. Attendance is optional and highly encouraged. There is an $80 fee per parent/guardian. Don't forget to book a hotel for your overnight stay visit for pricing and deals.
Parent/Guardian Information Form (Due May 31, 2013 - will be available by May 1)
The information shared facilitates parent/guardian involvement. For example, Student Affairs produces a monthly electronic newsletter especially for parents and guardians. The Campus Currents - Parents e-newsletter keeps parents/guardians apprised of general campus news and events. Parents/Guardians will automatically begin receiving the newsletter provided they submit their e-mail information.
Request For Reasonable Accommodation based on a Disability (Due May 31, 2013 or asap)

A student who wishes to be considered for reasonable accommodations at Fairfield University must identify him/ herself to Disability Support Services. How to complete: Access the form via the First-Year Student Checklist or fairfield.edu/student/adss_student.html. The form, along with supporting documentation, must be mailed or faxed (fax (203) 254-5542) to Fairfield University directly.
Immunization/Student Medical Report Form (Due June 1, 2013)
Students must complete the Immunization Requirement Form and have it filed with the Health Center prior to the start of classes. Immunizations, which are mandated by Connecticut state law, are highlighted on the Immunization Form for full-time undergraduates. Students may not move into a residence hall or begin classes until all immunization requirements are met. This form is required prior to students' arrival on campus.
How to complete: Have your student do the following: Download form via the First-Year Student Checklist or Parent/Guardian Checklist located at www.fairfield.edu/orientation. The form must be submitted by June 1. Mailed/faxed (fax: (203) 254-4263) directly to Fairfield University.
Financing College Expenses Payment Plan Enrollment (Due August 1, 2013, optional)
The University makes available a monthly payment plan administered by Sallie Mae. The last possible date for plan enrollment is August 1, and no applications or budget increases can be made after that date.
How to complete: Call Sallie Mae at (800) 635-0120 or visit tuitionpay.salliemae.com.
Health Insurance Waiver (Due August 1, 2013)
Fairfield University requires all full-time undergraduate students maintain or purchase a health insurance policy. This requirement was established to maintain the health and well-being of students, which is integral to the quality of their college experience. The University automatically enrolls each student in the University-sponsored health insurance policy for the upcoming academic year. A charge for this policy appears on the student's tuition bill. If the student has access to health insurance through other means (e.g., parent's policy) and does not wish to be enrolled in the University-sponsored plan, they must waive the university-sponsored plan in order to receive a full credit. Students coming from other geographic areas that carry HMO or network-based coverage are strongly encouraged to purchase the University-sponsored plan. How to complete: Have your student do the following: The health insurance waiver can be accessed via the First-Year Student Checklist or students can log in directly to my.Fairfield at my.fairfield.edu. Scroll to Fairfield Bookmarks, then click the Health Insurance Waiver link (at the top) and fill in the information using your health insurance card. Remember to click submit when done.
