Counseling & Psychological Services - Choices
You have been sanctioned to attend CHOICES, an alcohol education class conducted by Pam Paulmann, MPH in Dolan West, Room 3 (under archway). If you have class on Wednesdays during Choices, contact ppaulmann@fairfield.edu.
You have been given a pre-test that must be completed before your class. It must be turned into the Office of the Dean of Students or you must bring it to class. This is a class requirement.
After completion of this class, you will be given a Certificate of Attendance. It is your responsibility to provide this form to the Office of the Dean of Students.
Failure to complete this educational sanction by the assigned due date will result in additional sanctions.

When registering, include your name, student ID, year, box number, and the class day and time you wish to attend.
