Returning Student-Athlete Checklist

Each of the following forms and documents must be completed and returned to the Fairfield University Sports Medicine Department by all returning student-athletes at Fairfield University.

• Student-Athlete Insurance Form
• Follow-Up Health Questionnaire
• Acknowledgement of Insurance Requirements
• Copy of both the front & back of Insurance Card
