Student Visa Checklist

 

You will be notified by e-mail of the deadline. No exceptions or extensions. Students whose materials do not reach our office by deadline are responsible for obtaining their own visas.

Please submit ONE ORIGINAL and TWO PHOTOCOPIES of each of the following:

The Italian Consulate does not accept any documents with correction fluid, correction tape or cross-outs.

___ 1. Application Form (click here) completed, signed (with your signature notarized), plus one passport-sized photo. There is no designated area for the notary seal: just have the notary fit it on the signature page (USE RIGHT HAND COLUMN). The Consulate will not accept forms that have white-out or cross-outs. If you make a mistake, start over with a fresh form.

___ 2. Your Passport, valid for at least 6 months beyond your stay in Italy. Your passport will be returned with your visa. BE SURE YOUR PASSPORT IS SIGNED AND VALID FOR AT LEAST 6 MONTHS BEYOND YOUR STAY.

___ 3. A clear photocopy of your current student I.D. (enlarged, if possible).

___ 4. NON-Fairfield students ONLY: A letter from your home institution confirming that you are fully enrolled in the current academic year with your expected graduation date listed.

___ 5. Affidavit of Insurance (click here), completed, signed, and notarized. This form MUST be signed by the student. This form states that you will be purchasing Italian health insurance upon arrival in Florence.

Are you financially dependent on your parents? If so,

___ 6a. A notarized affidavit of support from your parents (click here) stating they will assume full financial responsibility for you during your stay in Italy (see sample letter).

___ 6b. A letter from your parents' bank, ON BANK LETTERHEAD, addressed to the consulate and signed by a bank official, indicating account balance (see sample letter). Bank statements will not be accepted. A minimum of $1000 per summer session ($2000 for both summer sessions) or $6000 per semester is required. Further financial information is not required. Are you financially self-sufficient? If so,

___ 6c. A letter from your bank, ON BANK LETTERHEAD, addressed to the consulate and signed by a bank official, indicating account balance (see sample letter). Bank statements will not be accepted. Instructions for Completing Visa Application Form

All materials must be submitted together. Items will not be accepted separately. Do NOT use regular mail; hand deliver to our office or use Fed Ex or other overnight service.

Complete each item fully. Do not leave any items blank. Incomplete applications cannot be processed.

  • For items that do not apply, such as name of spouse, province of birth, etc., write N/A in the space provided.
  • Notaries are usually available at banks.

NO CORRECTION FLUID, CORRECTION TAPE, SCRIBBLES OR CROSS-OUTS ON FORMS. IF YOU MAKE A MISTAKE YOU WILL HAVE TO RE-DO THE FORM.

3. First names - first name

11. Father's name - first name only

12. Mother's name - first name and maiden name

13. Type of passport - circle National passport

15. Issued by - this is found on the passport - put down the authority (ex: Stamford Passport Agency, US Department of State, etc.)

18. If you reside in a country other than your country of origin - write N/A US Citizen

19. Current occupation - student

20. Employer name and address - school and school address (your home institution)

21. Main Destination - Florence, Italy

22. Type of visa - circle Long Stay

23. Visa - circle Individual

24. Number of entries requested - circle Multiple Entries

25. Duration of stay - program dates only

26. Other visas - N/A

27. In case of transit, have you an entry permit? - Circle yes, until last day of program Issuing Authority - found on passport - put down the authority (ex: Stamford Passport Agency, US Department of State, etc.)

28. Any previous stay in this or other Schengen States - either put N/A or any previous trips you have taken

29. Purpose of travel - circle other and write down Liberal Arts Study Program

30. Date of Arrival - the first day of the program

31. Date of Departure - the last day of the program

32. Border of first entry or transit route - write down the first place you land in Italy

33. Means of transportation - aircraft

34. Name - write down Joanne Maddux

36. Means of support during your stay - circle Cash, Credit Cards

Then circle health insurance. Valid until: last day of the program

The rest are all N/A until #44.
