Bed Lofting Form
For students living in Dolan East, Gonzaga, and Loyola Halls who wish to have their beds lofted in one of five different configurations.

Commuter Request Form
For full-time undergraduate students of the University who wish to change their status from resident to commuter. In order to qualify for commuter status, you must be residing full-time with a parent or guardian and living within thirty-five miles of campus.

Housing Application
For all non-first year students wishing to reside on campus. First year and new students can find their housing application here.

Co-Op Form
This form should be completed by junior or senior students who are anticipating openings in their room assignments in the spring semester, which could be filled by friends that are studying abroad in the fall semester. This form is to be completed by the residents who will reside in the housing assignment for the entire academic year.

Post-Lottery Request Form
Should be completed out by students who did not fill out the housing survey.

Individual Responsibility Form
If there is any damage to your living space, please have the responsible person identify him/herself with this form, specifically citing the damage and its location. If any damage is found and no one claims responsibility, the charge will be distributed equally among all occupants of the living space.

Damage Appeals Form
This form should be filled out if you would like to appeal any damages that you were billed for at the end of the academic year, but you believe that you are not responsible for. Completed forms should be sent to residencelife@mail.fairfield.edu by September 5, 2008.
